DRIVER RECORD REQUEST - ANNUAL
Your Driver’s Record is required by our insurance company to determine if you are fit to drive during your shift for your client(s).
To pull your own Driver Record, you may go to the DMV in person, or you can request it online (directions below). If you would rather authorize us to pull this report for you, please email us with a request for us to process this for you. Make sure your request includes an authorization for us to pull this report and use it to meet our insurance requirements. Lastly, if you would rather come in to the office and we can pull the report together, just make an appointment with the front office at 707-496-0898.
DMV ONLINE REQUEST INSTRUCTIONS:
1. Sign in to the DMV website
2. Have your debit or credit card ready. The cost is around $2.00.
3. Once the report is ready, you can email it to us.
4. Make sure you submit the receipt for reimbursement.
