Please use the form below to submit incontinent AND medication supply orders for PACE Participants (ONLY). Order supplies one week in advance to allow time for shipment. Do not include orders for gloves or masks as PACE does not supply them for our agency (contact the Agape Administrative Office for gloves):

Diapers (size and style - pull up or fastener); Diaper Liners; Chux Pads; Peri-Wash; No-Rinse Body/Hair Wash; Wipes

This Participant needs the following medications and/or supplies:

Medications (PRN, Blister Pack, Insulin, Bottled. Patches); Ointments or Creams; Drops; Bandages; Diabetic Test Strips, Lances or Needles
